Responsibilities of an MDS Coordinator:

  • Assess patients'/residents' physical, psychological, and psychosocial health. This includes assessing treatments like dialysis, chemotherapy, and physical therapy etc.
  • Assist in developing care plans and goals for improving patient/resident health.
  • Monitor patients'/residents 'medical insurance and payment plans. Provide required updates to managed care contractors for continued services.
  • Manage documentation for patient/resident appeals.
  • Coordinate with healthcare team members to update care plans and track quality indicators as well as CMI.
  • Create and submit reports to state and federal governments on patient health at the facility.
  • Meet with patients'/residents' families and caregivers to discuss their condition and treatment plans.
  • Capture, coordinate and manage data related to patient assessments.
  • Works in collaboration with the MDS Team and Interdisciplinary Team to assess the needs of the resident
  • Assists with coordination and management of the assessment reference dates along with the rehabilitation site manager.
  • Complies with federal and state regulations regarding completion and coordination of the RAI process.
  • Monitors MDS and care plan documentation for all residents. Ensures documentation is present in the medical record to support MDS coding.
  • Discuss opportunities for capturing reimbursement with Medical director when appropriate.
  • Maintain current MDS status of assigned residents according to state and federal guidelines.
  • Maintain the frequent and accurate data entry of resident information into the MDS system.
  • Complete accurate coding of the MDS with information obtained via medical record review as well as observation and interview with facility staff, residents and family members.
  • Remain up to date on education regarding changes to PDPM, RAI manual, and CMI.
  • As directed may attend interdisciplinary team meetings and other meetings in order to gather information. Communicate changes and maintain and update records.
  • Continually updates knowledge base related to data entry and computer technology,
  • Completes electronic submission of required documentation to the State database and other entities per Brooke Grove policy.

Requirements of an MDS Coordinator:

  • Associate or Bachelor's degree in nursing from an accredited institution and at least two years of nursing experience in geriatric nursing and/or acute care nursing.
  • Must possess a valid nursing license issued by the State of Maryland to practice as a Registered Nurse.
  • Possess sufficient previous experience in clinical health care including direct experience in geriatric or acute care settings.
  • Must be knowledgeable of the MDS current assessment process. RAC-CT preferred.
  • 2 or more years of MDS experience is required.
