ALBANY ADVOCACY RESOURCE CENTER JOB DESCRIPTION SOURCE PROGRAM LICENSE PRACTICAL NURSE (LPN) JOB SUMMARY: The SOURCE Licensed Practical Nurse Full-Time (LPN) is responsible for working cooperatively with the Registered Nurse (RN) to complete the Material Data System and Appendix F and ensuring that the reports are accurate and completed in a timely manner. Salaried Exempt Position. DUTIES AND RESPONSIBILITIES Obtains initial referrals from the Intake Team to assess new members. Assesses new members in their homes using the Material Data System. Obtains corrected Face Sheets from the Intake Referral and uses them to reassess all members annually on the Material Data System. Ensures that all information on the Material Data System for each member is accurate. Contacts a new members Primary Care Physician for other diagnose which may qualify the member for SOURCE if the new member does not have triggers to qualify him or her for SOURCE. Attends all Medical Directors weekly meetings to discuss and sign off on the Appendix F. Enters the Medical Directors weekly case note in Therap. Obtains the Appendix Fs from the RN and faxes them to the Department of Community Health. Reviews the members functioning and health status with their Primary Care Physicians to ensure concurrence with Material Data Systems or information. Other duties as assigned. May be required to assist with nursing needs in the Residential Program as directed. Reasonable accommodation may be made to enable individuals with disabilities to perform the essential functions. QUALIFICATIONS: Required: A nursing license from an accredited institution. OTHER KNOWLEDGE AND ABILITIES: He/she must be a mature person who is friendly, caring and who has good communication and organizational skills. He/she must have a personal vehicle and a valid Georgia drivers license. He/she must have a working knowledge of the Albany Advocacy Resource Centers Mission and Vision as well as the agencys programs and services. He/she must have a knowledge and understanding of the agencys accreditation process and its value to the organization. TYPICAL WORKING CONDITIONS: When required to work in the office, the environment will be that of a typical office atmosphere. Frequent travel is expected as part of this job qualification. JOB RELATIONSHIPS: He/She will be supervised by the Co-Director of Nursing and the SOURCE Program Manager. He/she will have no supervisory responsibilities over other employees.
