Job Title: LMSW- Social Worker

Role Summary

Our client is seeking Social Workers for creating outcomes for the patient and family by managing complex psychological and economic co-morbidities. You will provide patients and families education and consultation regarding resources, collaborate to develop a discharge plan and facilitate and monitor its implementation.

Duties and Responsibilities

• Assess patient's and family's psychological risk factors though evaluation.
• Document in medical records patients identified as high risk for readmission.
• Provide intervention in cases involving child abuse/neglect, domestic violence, elder abuse, institutional abuse and sexual assault.

Qualifications and Skills

• Master Social worker with current licensure to practice in the State of Michigan (LMSW) or if LLMSW must obtain LMSW within 6 months of qualifications to sit for exam.
• 2+ years experience in short term acute care in a hospital setting preferred.
• Prior Case Management
