As we continue to grow, we’re looking for a Hospice Admissions Nurse who shares our vision and wants to make every patient’s first step into hospice one of reassurance and support.
As the first clinical face of hospice, you’ll be the steady presence patients and families lean on during one of life’s most vulnerable transitions. Your expertise will bring clarity. Your compassion will bring peace. And your care will set the tone for the entire journey ahead.
Conduct thorough clinical assessments to determine eligibility and needs—physical, emotional, and spiritual.
Partner with physicians and care teams to coordinate smooth, timely admissions.
Guide patients and families through what hospice is, what to expect, and how we’ll be there every step of the way.
Develop personalized care plans that reflect each patient’s unique circumstances and wishes.
Provide immediate symptom management and emotional support.
Ensure documentation is clear, timely, and aligned with regulatory standards.
Serve as a trusted, compassionate resource for patients, caregivers, and loved ones during a life-changing transition.
Current, unrestricted RN license in Arizona
Minimum 2 years of clinical nursing experience (hospice or home health preferred)
Strong assessment and communication skills, with a gift for empathy and connection
A calm, grounded presence that instills confidence in patients and families
Organizational skills and EMR proficiency
Familiarity with hospice eligibility criteria and CMS guidelines (preferred)
